POSITION TITLE: Case Manager

DEPARTMENT: Assertive Community Treatment Team (ACT)

GENERAL HOURS

OF WORK: Generally 8am-5pm Monday-Friday with occasional rotational on call stints

EXEMPT/NON EXEMPT: Non-Exempt

SUPERVISOR: ACT Team Lead/Mental Health Professional

POSITION SUMMARY: Under the supervision of the ACT Team Lead/Mental Health Professional, the Case Manager assists participating individuals who have serious and persistent mental illness in developing and maintaining viable living, working and social situations in the community by obtaining needed medical, social, psychosocial, educational, financial, vocational and other services.

ESSENTIAL FUNCTIONS:

  1. Provides provides service coordination (case management) for an assigned group of clients including coordinating and monitoring the activities of the individual treatment plan.

  2. Provide ongoing evaluation of recipient’s mental illness symptoms, current functionality, and the recipient’s response to treatment. Participate in the development of the Individual Treatment Plan (ITP), providing discipline specific interventions, while considering and promoting recipient centered goals.

  3. Utilizing illness management and recovery strategies and in collaboration with the ACT Team, provide ongoing evaluation, problem solving, side-by-side services, skills training, and supervision (e.g., prompts, assignments, monitoring, encouragement), and environmental adaptations to assist recipients with activities of daily living so that quality of life and successful community placement are maintained.

  4. Provide support and assistance to recipients in maintaining family and social relationships so that social isolation is reduced.

  5. In collaboration with the ACT Team, provide comprehensive Individual Dual Diagnosis Treatment services so psychiatric symptoms are reduced.

  6. Participate in professional development activities that maintain skills consistent with current standards of practice. Identify own needs for continuing education with particular emphasis on enhancing knowledge and skills in community-based mental health.

  7. Maintains professionalism and models team cooperation. Maintains a supportive attitude towards program’s Vision, Mission and Values. Maintains ongoing professionalism and dialogue with the team.

  8. Provide individual and group treatment in the office and in community settings in a stage-based treatment model that is non-confrontational, considers interactions of mental illness and substance abuse, and has client-determined goals.

  9. Available for minimal evening and weekend work as required by ACT Team.

OTHER RESPONSIBILITIES:

  1. Ability to travel as position requires.

  2. Performs other work-related duties as assigned.

  3. Is sensitive and responsive to management needs such as data collection and program evaluation.

EDUCATION, EXPERIENCE AND SKILLS REQUIRED:

  1. Must be 21 years of age and have the ability to meet the Minnesota Department of Human Service rules for qualifications of a Mental Health Practitioner.

  2. Must possess a valid Driver’s License and Auto Insurance.

  3. Experience working with adults diagnosed with serious and persistent mental illness.

REQUIRED COMMUNICATION/MATHEMATICAL/REASONING SKILLS:

  • Must possess the required level of skills and abilities to read, write, speak, and apply mathematical concepts and sound reasoning to successfully perform the essential duties and responsibilities of this job.

  • Must be able to perform basic operations on a computer within the Windows operating system.

  • This position requires problem solving skills, critical thinking skills, and the ability to multi-task.

REQUIRED PHYSICAL ABILITIES:

  • While performing the duties of this job, the employee is required to sit; use hands to finger, handle, or feel; and talk or hear.

  • The employee frequently is required to walk.

  • The employee is occasionally required to stand and reach with hands and arms.

  • The employee must regularly lift and/or move up to 10 pounds and occasionally lift and/or move up to 20 pounds.

  • Specific vision abilities required by this job include close vision, distance vision, color vision, peripheral vision, depth perception, and the ability to adjust focus.
