We are currently hiring a Registered Nurse II for our Gender Affirming Care Program based in Worcester. The Registered Nurse will collaborate with a health care team with a specific focus on gender affirming care in the delivery of quality primary medical care for patients of all ages, races, and ethnicities. Adhering to designated nursing procedures within the scope of training, assess, treat, and educate patients about various medical conditions. The gender affirming care team nurse will also coordinate care and provide advice and emotional support to patients’ family members. The Nurse is a member of the clinical practice team and works collaboratively with other team members in providing patient care.

Essential Functions:

  • Provide direct patient care within the scope of licensure including, but not limited to, patient assessment, administration of vaccinations, medications, and treatments.

  • Injection teaching and medication education for transgender patients.

  • Support with clinical documentation for prior authorizations for medications and for pre-surgical coordination as directed by provider.

  • Delegate appropriate patient care activities within the organization to ensure smooth patient flow.

  • Facilitate coordination of patient care outside the organization by activities including, but not limited to medication management, review of diagnostic test results, and follow up on outside services provided (i.e. ER visits, consults).

  • Perform POC testing, as necessary.

  • Document patient care in the EHR in accordance with health center and nursing standards.

  • Modifies patient care as age appropriate.

  • Adhere to organizational policies and procedures as they pertain to nursing practice

    •     Triage (in person or via telephone) patients who present with acute symptoms.

    •     Request medical interpretation services as necessary.

    •     Assist in training new nursing staff or precepting students.

  • Our health center requires all employees to have the most recent COVID-19 booster and the yearly flu vaccine.

  • Please note this position may require evening, weekend and/or holiday shifts on a rotating basis.

    Preferred Qualifications:

  • MA Registered Nurse License / LPN license.

  • Current Basic Life Support (BLS) certificate.

  • 2 years of external experience, 1.5 years of experience at Kennedy Community Health

  • Demonstrated experience and/or interest in working with transgender individuals.
