Visiting Health Services of New Jersey (VHSNJ), a partnership betweenHackensack Meridian Health (http://www.hackensackmeridianhealth.com/) andSt. Joseph's Health (https://www.stjosephshealth.org/) , provides a comprehensive continuum of home health care and hospice services, support, and resources to patients and families in Passaic, Bergen, and parts of Morris counties. Responsibilities

Responsible for the supervision and guidance of field staff to ensure quality of care for hospice patients and caregivers, as well as appropriate completion of Medicare required documentation. Serves as a role model, advocate, and motivator to help new nurses acclimate, maintains their competency in hospice nursing and shares clinical knowledge and expertise. The incumbent should demonstrate the qualities of an effective nurse mentor including strong hospice knowledge, organizational skills and time management, goal-orientation, ability to collaborate with mentees to develop a beneficial learning plan, strong interpersonal skills, the ability to maintain composure at all times, be encouraging and non-judgmental, and the ability to receive feedback.

  • Conduct supervisory visits with the nursing staff to observe the home interaction and ensure that all quality indicators are being met including medication reconciliation, wound care (proper assessment, measuring and correct treatment), pain assessment, adherence to infection control practices, home safety assessment, and family satisfaction. Ensure documentation is appropriate to clinical needs. Complete supervisory visit documentation.

  • Instruct and observe HHA supervisions conducted by the nursing staff to ensure competency.

  • Observe and instruct in the development and updating of nursing care plans.

  • Schedule one-to-one instruction with nursing staff who need remediation and document the learning plan.

  • Assist in the development of critical thinking skills and problem solving.

  • Support new and existing clinicians in documentation excellence, COP compliance, case management, time management, and scheduling.

  • Conduct chart audits to ensure continuous survey ready documentation in the Epic environment, identify clinician knowledge gaps, and communicate required corrections to appropriate staff.

  • Collaborate with MCPs on learning opportunities and support to staff.

  • Take the lead role in new hire orientation and facilitate scheduling, completion of competencies, and required on-boarding education.

  • Participate in performance improvement projects and provide data for completion of QAPI reports.

  • Assess family satisfaction during supervisory visits, assist in identifying trends for improvement.

  • Collaborate with Staff Development to effectively ensure that all clinical staff develop a comprehensive understanding of the fundamental elements of Hospice Medicare conditions of participation, reimbursement requirements, and the ability to navigate HMH policy and Lippincott procedure manuals.

  • Schedule weekly check-in meetings with the orientee and MCP.

  • Train experienced nurses to become nurse preceptors.

  • May be required to function as staff RN providing patient care in the field or hospital setting, inclusive of managing a caseload.

  • Other duties and/or projects as assigned.

  • Other educational projects as needed.

  • Adheres to HMH Organizational competencies and standards of behavior.

  • Lifts a minimum of 50 lbs, pushes and pulls a minimum of 50 lbs and stands a minimum of 6-8 hours per day.

Qualifications

Education, Knowledge, Skills and Abilities Required:

  • Associate's Degree in Nursing or Diploma Certificate in Nursing.

  • Minimum three (3) years clinical experience in hospice.

Education, Knowledge, Skills and Abilities Preferred:

  • Bachelor of Science in Nursing degree.

Licenses and Certifications Required:

  • NJ State Professional Registered Nurse License.

  • AHA Basic Health Care Life Support HCP Certification.

  • Certified Hospice and Palliative Nurse within 1 year of entering position.
