Flexible caregiver ready to step in when needed during the week Regular Daily In-Home Schedule (for reference only) - 8:30 am 10:30 am - 5:30 pm - 7:00 pm, - 10:30 pm 11:45 pm. Ideal Candidate - Experienced and compassionate with personal care and transfers - Familiar with the unique needs of individuals with cerebral palsy. - A lover of petsboth dogs and cats - Must have a reliable vehicle for transportation. - Favorable Caregiver Background Check Inspiring Tasks - Assist her in rising each morning to embrace the day ahead. - Help her unwind and settle into bed for a restful night - Provide compassionate personal care, including transfers, showering and toileting - Prepare light meals and maintain a clean, comforting environment. Community Horizons offers support for individuals enrolled in self-directed programs through Managed Care Organization or IRIS (Include, Respect, I Self-Direct) throughout Wisconsin. Self-direction gives program participants the choice over who provides services and how they receive them.
