Clinical Pharmacist

Novant Health is seeking a Clinical Pharmacist, to join our pharmacy team, to deliver optimal medication management across all dimensions of care, every time. The Clinical Pharmacist serves as part of the healthcare team at Novant Health by supporting authentic personalized relationships with patients and their chosen support system, collaborating with team members to assess, plan, implement and evaluate an individualized plan of care that promotes optimal health. The pharmacist accepts responsibility, authority, accountability and ownership for management and provision of care in accordance with current policies and procedures.
Essential functions include:
Oversees the daily operation of the pharmacy
Retrieves and evaluates relevant biomedical literature, formulates appropriate responses, and provides concise and timely response to drug information requests
Prepare and dispense medications in accordance with best practice, policies and state and federal regulations
Designs, recommends, monitors, and evaluates patient specific pharmacotherapeutic regimens
Clinical review and order verification of pharmacy prescriptions, including hazardous and non-hazardous medications.
Communicate with clinic providers and nursing to provide remarkable patient care
Demonstrate proficiency with equipment and software
Precept pharmacy residents (PGY1/PGY2) and pharmacy students
Participate in committees such as P&T, Excelera Network sub-groups and shared governance
Medication therapy management and patient counseling
Promote a culture of quality and safety and adhere to policies and procedures relating to Quality Improvement Program

Qualifications
Education: 4 Year/Bachelor's Degree in Pharmacy, required. Doctorate of Pharmacy, preferred.
Experience: Postgraduate year 1 residency or fellowship OR two years of experience as a registered pharmacist in health system pharmacy, required.
Licensure/Certification: Current Pharmacy licensure in appropriate state(s) or must be completed within 30 days of start date by either examination or reciprocity, required. Board of Pharmacy Specialties Certification or other approved certification must be obtained and continuously maintained within 2 years of start date as applicable to the role; excludes PRN team members who may not be eligible to take certification exam, required.
Additional Skills (required): Other duties as outlined in Novant Health and facility specific policies and procedures. Ability to drive/travel to multiple facilities/locations as needed. Ability to work as part of a high performing, change ready and resilient team.
