DESCRIPTION OF WORK This Aging Care Manager 1 is a trainee position with a probationary period of six months and is specifically trained to assess the needs of consumers utilizing state mandated assessment forms, develop effective care plans to meet these needs, and implement these care plans in a professional manner. You will be responsible for completing Reports of Need on consumers aged 18 and older, after successful completion of required training courses. Work also includes making priority home visits for safety and welfare checks of consumers and working as a liaison between the consumer and provider.

This position is also responsible for assisting consumers, guardians, and families in maximizing their abilities for self determination by enabling and empowering their decision making to the greatest extent possible. You will reassess consumers on an annual or bi-annual basis, depending upon the care program the consumer is enrolled in. This involves identifying changes in situation or function, measuring progress toward goals, and educating consumers and their families regarding aging services and benefits.

Work Schedule and Additional Information: Full-time employment Work hours are 8:00 AM to 4:00 PM, Monday - Friday, with a 30-minute lunch. QUALIFICATIONS Minimum Experience and Training Requirements: Three years as an Aging Case Aide 2; or A bachelor's degree which includes or is supplemented by twelve (12) credits in social sciences, behavioral sciences, human services, or a closely related field; or An equivalent combination of experience and training. Applicants will be considered to have met the educational requirements once they are within 3 months of graduating with a qualifying degree.

Hiring preference for this vacancy may be given to candidates who live within Luzerne or Wyoming counties.
