Sagility has more than 25,000 employees across 5 co Job title: Care Coordinator - Clinical Operations Job Description: The Sagility Team is currently hiring a talented Care Coordinator to provide outreach services via phone and other engagement channels to complete a Social Determinants of Health (SDOH) screening assessment. Based on the SDOH assessment results, referrals are made to the clients in-house clinical social work team regarding areas such as medication adherence, annual wellness visit scheduling and flu vaccine gap scheduling and other applicable care needs as identified. Care Coordinators (Customer Service Advocate CSA) will document all inbound and outbound inquiries according to client direction. CSAs will take appropriate action to resolve inbound and outbound Inquiries according to client direction via telephone or other client approved engagement channels. In addition, the CSAs will maintain a professional, courteous and positive demeanor when interacting with customers. If additional questions arise that are out of scope for this engagement the CSA will transfer the customer to the appropriate client area for assistance. Knowledge and Experience: High School Diploma or equivalent 1 plus years of experience preferred in accessing community resources to help customers with basic needs Excellent communication and interpersonal skills with a focus on customer service and critical thinking Ability to adapt to change and problem solve Strong time management and organization skills with an ability to set priorities in a fast-paced environment Ability to utilize and navigate multiple technology systems Knowledge of managed care preferred Bi-lingual Spanish and English language proficiency required Hardwired internet connection with speeds greater than 5MB upload and 10MB download Secure, private work area at home Salary Range: $19.00 hourly This is a part-time role with hours scheduled falling between 9AM to 7PM Eastern Time, there are NO weekend shifts.
