University of Virginia Clinical Soc Worker-Dialysis in Waynesboro, Virginia
The BSW social worker care manager within the inpatient setting and in conjunction with the care management team screens and assesses patients to identify their medical psychosocial and financial needs and appropriate services. The discharge process for the patient begins on day one of patient’s stay. They will communicate with payers to facilitate the patient’s effective and efficient movement through the health care system. A directory of current information regarding community resources and eligibility requirements is maintained. Completes the Uniformed Assessment Instrument (UAI) and other government forms (local state and federal) as appropriate to access available benefits and services for patients. For UM purposes maintains expert knowledge of Interqual Level of Care Criteria.
Conduct a comprehensive care management assessment of each patient within 24 – 72 hours of admission. Identify patient’s readiness to discharge based on outcomes.
Complete continued stay reviews as needed to ensure patients are progressing towards discharge without barriers
Review all regulatory requirements to ensure high quality safe and cost effective care Identify patients with Length of Stay concerns for presentation at Transitional Care Hospital Utilization
Management Length of Stay meeting based on experience discharge barriers.
Acts as patient advocate.
Explores patient/families current and projected financial resources for healthcare and essential needs (employment SSDI SSI Workers Compensation STD LTD and FMLA).
Evaluate and measure outcomes in order to continuously improve practice and to enhance outcomes
Participates and assists in departmental projects as appropriate.
In addition to the above job responsibilities, other duties may be assigned.
Position Compensation Range: $26.23 - $41.97 Hourly
MINIMUM REQUIREMENTS
Education: Bachelor's degree in Social Work from a CSWE accredited social work program required.
Experience: 3 years relevant experience. Acute care setting preferred.
Licensure: Certification in Care Management required within 3 years of hire.
PHYSICAL DEMANDS
Job requires standing for prolonged periods, frequently bending/stooping/traveling and reaching (overhead, extensive, and repetitive). Proficient communicative, auditory and visual skills; Attention to detail and ability to write legibly; Ability to lift/push/pull 50 – 100lbs. May be exposed to chemicals, humidity, heat, blood/body fluids, infectious disease and dust.
