Molina Healthcare Community Behavioral Health Liaison in Warren, Michigan

JOB DESCRIPTION

We are looking Community BH Liaison who must reside in MI and will work in remote setting supporting Medicaid Michigan Population. This role will work closely with our health partners and will coordinate care gaps between Mental Health Services and Physical Health (Chronic Condition) management. We are looking for candidates that have experience working with inpatient/outpatient mental health patients. This role will have you completing assessments, creating care plans, and connect to community resources. Excellent computer skills and attention to detail are very important to multitask between systems, talk with members on the phone, and enter accurate contact notes. This is a fast-paced position and productivity is important. We are looking for experience working with variety of mental health issues as well as Substance Abuse.

Local travel into our office (Troy MI) may be required (Team Meetings or Trainings)

Home office with internet connectivity of high speed required.

Schedule: Monday thru Friday 8:30AM to 5:00PM EST

Job Summary

Molina's Behavioral Health function provides leadership and guidance for utilization management and case management programs for mental health and substance use services and assists with implementing integrated Behavioral Health care management programs.

Job Duties

• Serves as liaison between plan and community Mental Health and Alcohol and Drug agencies/services to facilitate the care coordination process and to ensure the provision of Behavioral Health (BH) as a benefit of managed care.

• Participates in community BH meetings that include the plan, County/Health Care Authority Services, county-based BH organizations, and others to ensure program integrity, and identify areas for development.

• Ensures adherence of the terms of the MOUs between plan and community service organizations.

• Serves as community liaison to BH advocacy groups, non-contracted community partners, service organizations, etc. specific to BH. Assists in community outreach activities to improve awareness of Molina Healthcare’s involvement in BH service management and health promotion.

• Assists with development and implementation of policies and procedures for referral process, care coordination, and other administrative and clinical processes as needed.

• Develops and facilitates plan training on BH and community service organization/county agency training on Managed Care.

• Collaborates with UM and CM Directors/Managers in coordinating clinical consultation of BH cases as part of the integrated care coordination team.

JOB QUALIFICATIONS

REQUIRED EDUCATION:

Bachelor’s Degree or equivalent experience

REQUIRED EXPERIENCE/KNOWLEDGE, SKILLS & ABILITIES:

Min. 3 years social work/behavioral health experience in an outpatient, inpatient setting, or managed care setting; behavioral health care coordination; knowledge of county mental health systems and of substance abuse conditions and treatment modalities. Knowledge and practice of variety of harm reduction principles and strategies. For specific markets, “lived experience” may be required.

REQUIRED LICENSE, CERTIFICATION, ASSOCIATION:

Must be Certified to practice in specific State

PREFERRED EDUCATION:

Master’s Degree in Social Work, Psychology, or other Behavioral Health field

PREFERRED EXPERIENCE:

3-5 years post-graduate experience

Experience coordinating care in higher levels of BH treatment such as Intensive Outpatient or Partial Hospitalization Programs, inpatient acute psychiatry, and substance abuse residential facilities.

Extensive knowledge of county mental health/substance abuse systems and social support resources. Extensive knowledge of harm reduction principles and tactical strategies.

PREFERRED LICENSE, CERTIFICATION, ASSOCIATION:

Active and unrestricted Licensed Clinical Social Worker (LCSW), Licensed Professional Counselor (LPC or state equivalent), Licensed Marriage and Family Therapist (LMFT), or Certified Peer Support Specialist, Certified Family Support Specialist, or Certified Community Health Worker.

STATE SPECIFIC REQUIREMENTS:

For specific markets, “lived experience” may be required.

PHYSICAL DEMANDS:

Working environment is generally favorable and lighting and temperature are adequate. Work is generally performed in an office environment in which there is only minimal exposure to unpleasant and/or hazardous working conditions. Must have the ability to sit for long periods. Pay Range: $21.82 - $42.55 / HOURLY
