: Agency Nurse (LPN) : Nursing


SUPERVISOR: Executive Director JOB SUMMARY: The Agency Nurse is responsible for overseeing the operation of the department and will provide nursing care to consumers, following established policies and procedures of the agency and assist in the care and management of all consumers. ESSENTION FUNCTIONS:

  1. Report to work as scheduled; notify supervisor as soon as possible if you will be absent or late.
  2. Maintain a current appointment schedule.
  3. Ensuring timely medical follow up as ordered.
  4. Ensuring MARs are updated and current at all times. This includes paper MARs as well as E-MARs
  5. Ensuring medications are ordered and available within 24 hours of initial order as well as ordering medication refills as needed.
  6. Communicate with agency pharmacy on medication orders.
  7. Communicating all medical information/results to team members.
  8. Ensure physician orders are signed and scanned to Therap timely.
  9. Assist Residential Coordinators with obtaining adaptive equipment prescriptions as needed.
  10. Maintain confidentiality regarding consumers and staffing issues.
  11. Monitor and document all medication errors.
  12. Monitor and document all Certified Medication Technicians and recommend training as needed.
  13. Establish and maintain effective working relationships with all departments.
  14. Escort consumers to medical appointments as needed.
  15. Include Residential Director on all communication with Residential staff.
  16. Monitor and assist as needed with ordering medications.
  17. Assist with review of turn around medications to ensure accuracy prior to sending to consumers residence/Day Services.
  18. Assist with review of mediations in general to ensure accuracy prior to sending to consumers residence/day Services.
  19. Maintain a secure area for all control medications.
  20. Keep all training current.
  21. Ensure/Monitor that all medical information if filed in hard copy chart as well as scanned into Therap timely.
  22. Maintain nursing vehicles and documentation as required by the agency.
  23. Create and maintain accounts for all consumers with Lab Corp.
  24. Assist with contacting Station MD as needed for consumers that become ill/injured while at Day Services.
  25. Ensure medication is provided for Day Services.
  26. Review monthly paper/e-mars to ensure all information is current.
  27. Review monthly paper/e-mars for medication errors.
  28. Review monthly/quarterly physicians orders to ensure all information is current.
  29. Document corrections to paper MAR/Therap MAR/physicians orders as needed to include transcribing new mediations and discontinuing medications.
  30. Review and document on all GERs.
  31. Contact Nurse Consultant for guidance and direction as needed.

ADDITIONAL RESPONSIBILITIES:

Perform all duties and responsibilities in a timely and efficient manner with established agency policies and procedures.

Maintain a favorable working relationship with all other departments to promote a cooperative and harmonious working climate to maximize employee morale, productivity and efficiency.
