Ridgeview Medical Center Occupational Therapist Home Care - Casual in Waconia, Minnesota

Purpose of Job This casual position is responsible for the delivery of individualized assessment, care planning, intervention and discharge of Occupational Therapy clients through the Home Health agency or Rehab Specialties department, working within the professional scope of practice as defined by the American Occupational Therapy Association. Job Functions Performs discipline specific evaluations, OASIS assessments, care planning, interventions, documentation of care, and discharge of client cases according to RMC, agency, and professional standards. Provides current and quality treatment, utilizing Ridgeview, agency, and professional resources to promote continued quality care. Communicates with the client, health care providers, insurer and others to ensure that client needs are addressed and authorization is obtained from appropriate parties for all visits made. Serves as client advocate to obtain referrals to advise and utilize appropriate Ridgeview, agency and community resources to meet client needs. Exhibits working knowledge of hours of operation, customer base, department operations, visit procedures, and chart audit procedures. Demonstrates competencies in the specialized area of Home Health, and utilization of reimbursement and regulatory guidelines for the provision of safe and effective care. Provides effective supervision of COTAs, home health aides, and other applicable caregivers when occupational therapy is provided. Participates in meetings, care conferences, and quality improvement initiatives as directed. Provides professional and courteous customer service to internal and external customers. Performs other duties as assigned. Minimum Education/Work Experience See License/Certification Knowledge/Skills/Abilities Ability to work independently and organize tasks. Ability to exhibit flexibility, self-control and maturity in work and behavior. Ability to serve patients in a friendly and efficient manner. Ability to respond appropriately to unpredictable situations and diverse patient populations. Ability to work autonomously and as a team. Capable of making independent decisions with the use of good critical thinking and problem solving skills. Ability to promote Home Health and Rehab Specialties Services. Knowledge of basic computer functions and Microsoft Office applications. Ability to communicate in the English language for effective written and verbal correspondence in order to complete job functions as mentioned above. License/Certifications Licensed Occupational Therapist, valid in Minnesota. Current Healthcare Provider CPR Certification Valid MN Driver's license and meets Ridgeview's age and insurability requirements.
