Description

San Mateo County Health's Aging and Adult Services Division is seeking qualified individuals for the position of Social Worker II (SW II) to provide a wide variety of services in such areas as information and referral, supportive services, protective services, out-of-home services, health related services, and other welfare within San Mateo County Health and the Human Services Agency.

The current vacancy is in the Enhanced Care Management (ECM) / Partners for Independence (PFI) Unit - Community Services (CS) that provides CS services coordination for adults who have complex medical and psychosocial needs and are at-risk for out of home placement. ECM/PFI CS is a voluntary program that may be offered to San Mateo County residents who have Medi-CAL, who are aged and/or disabled with CalAIM CS needs.

Distinguishing Characteristics

The Social Worker II screens applicants’ eligibility for CalAIM CS services. The SW II will travel throughout the County to conduct home visits to educate eligible clients regarding services, participate at Interdisciplinary team (IDT) meetings, coordinate services to assist clients and to perform other duties of the position.

The ideal candidate will possess the following:

  • Master's degree in Social Work.

  • Excellent communication skills with seniors, cognitive and behavior challenged clients and their caregivers to develop an appropriate respite schedule for clients.

  • Experience using comprehensive assessment tools to identify the needs of CalAIM CS applicants/participants.

  • Understanding of human development and behavior within diverse populations.

  • Ability to independently develop and implement sound case plans of action.

  • Experience balancing client-centered care coordination and the accompanying administrative documentation.

  • Familiarity in implementing program regulations.

  • Superior organizational skills to manage a large caseload and related documents.

  • Proficient computer skills to use several independent data base systems as well as Microsoft Excel and Word.

  • Ability to quickly learn and adopt new technology, policies, and procedures.

  • Excellent writing/documentation skills.

  • Valid CA driver license is required.

  • Fluency in other languages in addition to English is a plus.

Examples Of Duties

Duties may include, but are not limited to, the following:

  • Perform outreach to provide education and information about CS services to individuals who may be at risk and unaware of services; inform clients and community members of programs and services.

  • Provide education and coordination for CS services to the clientele served:

  • Assist in planning for a schedule; identify potential respite or personal care and homemaker workers; arrange contract agency and assist in the facilitation of programs and activities to support client health/wellness.

  • Develop rapport, establish, and maintain contact with members of the community served.

  • Monitor and track program services that client received and make adjustments to meet client’s needs.

  • Provide guidance and advice to client on services usage.

  • Coordinate services schedule with client and provider.

  • Assess and reassess client’s need for CS services.

  • Complete and submit Services Request form for CS services for clients.

  • Participate at Interdisciplinary team (IDT) meeting to assist to develop care plan for clients.

  • Coordinate and communicate with providers and internal staff on CS services claims.

  • Assist Managers in CS services billing to HPSM.

  • Complete necessary documents, paperwork, and work within different information technology systems.

  • Provide advocacy for client and family/natural supports.

  • Prepare reports, case records, and correspondence.

  • Participate in quality improvement efforts.

  • Engage in activities to support San Mateo County's vision for integrated services.

  • Collaborate and facilitate service with care providers and other partner agencies.

  • Prepare various reports and correspondence; maintain accurate case notes; maintain caseload to meet the outcome requirements of funding sources; maintain files related to client relations, program operations, and other activities.

  • Perform special assignments and projects and other related duties as required.

Qualifications

Licensure/Certification:

  • California class C driver license or equivalent.

Education: Satisfactory completion of 30 semester units of social welfare and/or appropriate behavioral science courses.

Experience: Two years experience performing duties comparable to those of a Social Worker I or higher in San Mateo County. Substitution: Possession of a Master's Degree in Social Work from an accredited school of Social Work.

Knowledge of:

  • Principles of individual, group, and community behavior.

  • Scope and nature of current problems in the field of social work.

  • Social work methods and procedures.

  • Social service programs, and State and Federal legislation.

  • Techniques and principles of interviewing and counseling and problem solving methodology.

  • Social and community resources and awareness of how to identify, develop, and properly utilize these resources.

Skill/Ability to:

  • Understanding and implementing the agency's programs, policies and procedures.

  • Implementing caseload management techniques.

  • Establishing rapport with clients and their families in working toward solutions to their problems.

  • Analyzing problems and needs of clients and developing courses of action which lead toward constructive solutions.

  • Speaking and writing effectively.

  • Establishing effective working relationships with other employees.

Open and Promotional. Anyone may apply. Current County of San Mateo and County of San Mateo Superior Court of California employees with at least six months (1040 hours) of continuous service in a classified regular, probationary, and extra help/limited term positions prior to the final filing date will receive five points added to their final passing score on this examination.

The examination process will consist of an application screening (pass/fail) based on the candidates' application and responses to the supplemental questions. Candidates who pass the application screening will be invited to a panel interview (weight: 100%). Depending on the number of applicants, an application appraisal of education and experience may be used in place of other examinations or further evaluation of work experience may be conducted to group applicants by level of qualification . All applicants who meet the minimum qualifications are not guaranteed advancement through any subsequent phase of the examination. All examinations will be given in San Mateo County, California and applicants must participate at their own expense.
