On With Life Person Served Nurse Care Coordinator in United States

Our Person Served Nurse Care Coordinators (PCC) primarily work Monday-Friday 8am to 4:30pm, approximately 40 hours per week. This position also participates in a leadership on-call rotation. IMPORTANT NOTE : Please be aware that our orientation process (typically up to 1 month) may require working days or shifts outside of what will be your permanent schedule.

As a Person Served Nurse Care Coordinator (PCC) at On With Life, you can be a part of something greater. The rehabilitation process from a brain injury or other neurological condition is a 24/7 job for each person served, and our nurses collaborate with our medical providers, dietary team and other individuals in our transdisciplinary team of experts to keep persons served healthy along the way.

What do Person Served Nurse Care Coordinators (PCC) do?

Under the direction of the Director of Nursing and in collaboration with other members of the rehabilitation team, the PCC is assigned a specific set of persons served for which the PCC has the following responsibilities:

  • Participates in leadership on call schedule as assigned.

  • Reports changes in conditions of persons served to physician and responsible party

  • Updates Care Plans based on significant changes in person served status.

  • Attends person served care conferences, and therapy rounds as scheduled.

  • Assists with prior authorizations

  • Acts as wound nurse

  • Fosters strong relationships with PS, families, staff and interdisciplinary team

  • assists in education, monitoring and coaching the nursing staff on continuous quality improvement issues, and assists in implementing nursing policies and procedures

  • collaborates with other rehabilitation team members, such as Therapy, Case Management, & Dietary employees, to ensure comprehensive care

Qualified applicants must have:

  • Current licensure in good standing to practice as a Registered Nurse in the State of Iowa, CRRN preferred

  • At least 2 years of experience as a nurse

  • Leadership experience preferred

  • Ability to maintain CPR certification
