Essential Functions of the position are detailed below, and include any physical requirements below that. 1. Provides immediate screening of incoming client calls and internal program referrals for the Behavioral Health Department and enters the information into the Electric Health Record. Completes the assessment process for all clients. 2. For clients who do not qualify for services, provide referrals for services either within Catholic Charities or to an external partner.3. Maintains up-to-date and accurate documentation and database as required.4. Verifies appropriate documents for insurance and sets fees if a client does not have insurance. Prepares the Medicaid process for clients if they are eligible. Ensures information is submitted according to program protocol and CCAC policy.5. Follows established process for successfully onboarding clients including scheduling appointments and ensuring client charts are accurate and complete. 6. Responsible for the accuracy of all data entered into the Electronic Health Record and follows up with managers for corrections as needed. Leads all Agency PQI functions including database reporting, outcomes reports, client success stories, case record reviews, and client satisfaction surveys.7. Complete other duties as assigned. PLEASE NOTE: Essential functions include all other duties and responsibilities as assigned. X Kneel and move from sitting, bending, kneeling, or standing multiple times a day. X Push and pull objects up to 10 pounds.X Climb up and down up to 3 flights of stairs at a time. X Lift up to 10 pounds.
