Helio Health Inc. Senior Case Manager in Utica, New York

Overview

To support the mission of Helio Health, Inc., the Senior Case Manager will provide Case Management and coordination of services to patients/residents participating in, or who have completed other programs while supporting the program staff and Program Director with the day-to-day operations of the program.

In addition to our comprehensive benefits package, Full-Time colleagues, this position is eligible for a Sign-On incentive up to $1,500!

Pay: $24.03 - $26.70 per hour.

Responsibilities

  • Maintains a case load and performs primary case work responsibilities to include the following:

  • screening

  • intake

  • orientation

  • assessment, evaluation, and intervention

  • referral

  • treatment planning

  • counseling

  • crisis intervention

  • patient/resident education

  • case management

  • reporting and record keeping

  • consultation with other professionals

  • coordination of care with other community providers

  • discharge planning

  • Assists with training of staff, volunteers and students as assigned.

  • Performs Quality Assurance activities to include:

  • Provides services according to agency policies, procedures, program guidelines and governmental regulations.

  • Monitors provision of services when assigned.

  • Evaluates programs and identifies strengths/weaknesses.

  • Assists in problem resolution and implementation of supervisor approved program modifications.

  • Participates in program administrative and clinical committees as assigned.

  • Represents agency to the community.

  • Other duties as assigned.

  • Data Compliance.

  • Chart Audits.

  • Acts as Supervisor in the absensce of Program Leadership

  • Attend comminity meetings as needed.

Working Relationships

  • Takes direction from the Team Leader and/or Program Director and keeps them informed regarding content of workload.

  • Consults with team members working with participating patients/residents.

  • Ensure adherence to Quality Assurance requirements.

  • Works with patients/residents, internal and external community agencies and organizations.

  • Coordinates care and services with external case managers and managed care entities (operators) when necessary.

  • Works directly with medical and psychiatric providers regarding client needs, appointment scheduling and/or appointment cancelations.

  • Works directly with prescribers and pharmacies as it relates to prescriptions required by residents, refills of prescriptions and/or concerns with medications prescribed.

  • Works directly with referral sources and discharge sources.

Qualifications

  • Qualified Health Professional preferred (Q.H.P).

  • (including but not limited to LMSW, LCSW, LMHC, RN, NP, etc.) or Credential (including but not limited to CASAC, CPP, CPS etc.)

  • Bachelor’s degree preferred; Associates required.

  • Two-years of experience in the Human Services field.

  • An equivalent combination of training and experience may be substituted for the educational and employment requirements.

  • Must demonstrate steady progress towards Qualified Health Profession status if not currently Q.H.P.
