Upstate Cerebral Palsy Licensed Practical Nurse (LPN) in Utica, New York
Description
Pay $26.00 - $27.82 an hour
Seeking positive, motivated individuals to assist adults and children with mental illness and substance use disorder in our outpatient clinic. The Licensed Practical Nurse (LPN) will function as an integral part of a treatment team. This position works collaboratively with providers, clinicians, and other support staff to ensure that clients are holistically cared for. The LPN participates in the care of the client by gathering health information, providing long-acting injections to clients, assisting with collection of specimens, scheduling appointments, coordinating referrals, and medication refills. The LPN will also assist the crisis team as needed during times when clients are in crisis.
Core Responsibilities
Work on a positive Team to develop good practices and develop strategies to support individuals with mental health needs.
Assists to coordinate provider schedules and obtain controlled substance reports.
Provide nursing care to clients including ensuring that documentation is complete and medication lists are current.
Administers long-acting injections to clients as ordered.
Collect specimens, document and retrieve lab results.
Monitor incoming lab, consultation reports and telephone calls to ensure that Providers are updated.
Ensure that prescriptions are sent into pharmacy as ordered.
Monitor all medications onsite.
Complete all required documentation and reports according to program regulations including program participants’ charts.
Ensure compliance with all pertinent government and agency regulations and operating standards
Qualifications
A.A.S. Degree or High School Degree or equivalent plus Specialized Trade Certificate.
Current NY State LPN license and registration.
Experience working with the substance abuse population preferred.
1-3 years’ experience.
Valid NYS Drivers License.
