Provide critical services to students with disabilities ages 14 to 21 not receiving DRS-VR services to prepare them for employment. Parental permission is a must.

Job Duties:
- Verify student eligibility*
- Assist students in making an ILP - Independent Living Plan*
- Assist in identifying specific resource needs of consumers.*
- Maintain and develop Fast Track school referral contacts *
- Market the Fast Track Program
- Collaborate with youth agencies*
- Develop and obtain student referrals through the Department of Rehabilitation
- Secure competitive integrated employment with appropriate lawful wage compensation.
- Provide students & their parents information on rights/responsibilities *
- Other duties as assigned.
Assist consumers in moving out of a nursing home into the community
Job Duties:
- Cultivate working relationships with nursing home staff and with housing providers *
- Assist consumers in the development of an Individual Reintegration Plan*
- Assist in receiving deliveries of, moving, purchasing furniture/household items arranging transportation, acquiring adaptive equipment, etc.*
- Balance checkbook regularly with the Administrative and Financial Assistant
- On-call emergency follow-up regarding the safety of consumers moved for 6 months
- Participate in satisfaction surveys and determine outcomes
- Act as Facilitator for consumers served *
- Achieve grant goals pertinent to the position*
- Document services/progress/compile quarterly reports/monthly expenditures/flow chart billings
- Other duties as assigned.
