Providence St. Joseph Health Clinical Liaison - Social Worker or LPN in Tukwila, Washington

Description This position will cover King and Pierce county (primary focus on Central and North Seattle). The Clinical Liaison position is responsible for creating and maintaining positive customer relationships in order to increase referrals for Home Health, Hospice, and other services as needed. This role includes working with discharge planners, facility staff, physicians, and individuals to assess appropriateness for hospice and home health services, and facilitate the coordination of services and to ensure there is appropriate documentation for compliance with agency and statutory guidelines to manage the transition of the patient from an acute care facility to the patient's home or next location of residence. Required qualifications: Education sufficient for clinical licensure for discipline (LPN/LVN) Or Bachelor's Degree in Social Work Upon hire: Current state professional license or certification appropriate to discipline. 3 years of experience in clinical discipline. Preferred qualifications: Master's Degree in Social Work Bachelor's Degree in Applicable clinical discipline (e.g., Respiratory Therapy). Liaison, marketing or sales experience. Health care industry experience. Documented prior successful sales account medical management experience. Epic EMR experience. Why Join Providence?
