NEW AMERICANS HEALTH NAVIGATOR - FULL TIME TROY, MICHIGAN
BI-LINGUAL IN Ukrainian REQUIRED
We are adding a Refugee Health Navigator to our team who will identify and develop community mental and physical health partnerships and services and communicate external support mechanisms to refugee families. This integral part of our team will assist refugee families in assessing their children's needs with available community resources and help families bridge cultural and logistical barriers to mental and physical health.

Duties and Responsibilities
Identify and collaborate with partner providers delivering high quality, trauma-informed physical and mental health care for refugee youth, and to improve access to treatment
Identify and connect youth with healthy nutrition, physical recreation and extracurricular activities in their neighborhoods/schools; begin creating a resource list of trauma-informed physical health and mental health providers
Intervene early with community-based interventions for vulnerable children to help reduce emotional, social and psychological stressors and lower the risk of individuals developing more serious mental health concerns
Assist in bridging language barriers to access cultural sensitivities and the stigma of mental health services that may prevent youth from participating in essential treatment
Advance culture diversity training with area medical partners through a patient based learning approach, creating increased cultural sensitivity and competency opportunities
Match youths needs to services, assist with logistics, work with families, and directly connect children to appropriate services
Collaborate with referral sources, agencies, institutions, and individuals who have important roles in client's health
Conduct RHS-15 mental health screening for all referred clients and make timely, appropriate referrals to mental health care providers
Complete all appropriate documentation and reporting in accordance with required timeframes; maintain same in centers case files
Engage in careful record keeping with regards to referrals, appointments, and medical plans for clients. Adhere to HIPAA standards and ensure confidentiality
Enroll and coordinate youth in evidence based expressive arts/movement therapies
Share Health and Wellness Navigation learnings with other local resettlement agencies and office of Global Michigan
Provide written and oral education about USCIS TB testing and immunization requirements, answer questions, respond to concerns regarding immunizations and refer to a public health nurse when needed. Assist clients with completing their USCIS immunizations and attestation requirement within 90 days of arrival (if necessary). Assist SRHC and MDHHS TB program with TB evaluation and LTBI data reporting
Provide written and oral education about the refugee medial screening (RMS) process and components. Coordinate with the local RMS provider for scheduling RMS appointments with regards to scheduling appointments, transportation and interpretation services
Job Qualifications:
Education, Training, and Licensure/Certification
Bachelor degree in social work, medical case management, public health, nonprofit administration or related field;OR
A combination of education and work experience in Social Work, Medical Case Management, Public Health, Nonprofit Administration or related field
Experience
Minimum one year in refugee related casework and/or health or community services
Valid unrestricted driver's license with good driving record required. Reliable personal transportation suitable for safely transporting persons served; proof of current registration and automobile insurance required
