Join our Populance team as the Director Platform Success, where you will play a pivotal role in shaping the future of population health programs. Populance is a start-up organization established in 2023 providing industry-leading Care Management, Utilization Effectiveness and Population Health Analytics and Insights services. This start-up organization, being established with HFH/HAP, will serve both internal and external provider organizations to help them succeed and thrive in value-based payment models and population health programs.

This position is based in Detroit, MI with a preference for candidates located within a commutable distance who are open to a local hybrid work environment. We are open to full hybrid/remote, but candidates must be open to monthly travel to Detroit as needed or client sites.

Populance is seeking a dynamic and experienced Director of Platform Success to lead the management, configuration, integration, and optimization of vendor applications essential for the population health and value-based care analytics platform. This role oversees the full lifecycle of Populace’s vendor platform applications, from selection and implementation to ongoing optimization, ensuring alignment with customer needs.

Responsibilities include data management and integration, encompassing validation, transformation, and onboarding tailored to each customer’s requirements. The Director will hold vendors accountable to contractual deliverables and requires strategic leadership, healthcare technology expertise, attention to detail, and strong project management skills for large-scale initiatives.

Key Responsibilities

Vendor Application Management and Implementation

  • Lead the selection, configuration, and implementation of vendor applications aligned with value-based care objectives and population health management.

  • Cultivate and maintain strong vendor relationships, negotiating contracts and ensuring compliance with service-level agreements (SLAs).

  • Collaborate with healthcare providers and clients to customize applications to meet specific patient care and population health goals.

  • Partner with service delivery teams for effective onboarding, integration, and application support, focusing on client satisfaction and improved health outcomes.

    Data Management and Integration

  • Develop and execute a comprehensive data integration strategy to ensure accuracy, consistency, and accessibility across critical systems.

  • Lead data integration projects connecting vendor applications with clinical systems to enable seamless data exchange and enhance usability.

  • Establish data governance policies to maintain quality, privacy, and security while complying with healthcare regulations (e.g., HIPAA).

  • Collaborate with IT, compliance, and client teams to meet industry standards for population health management.

    Service Delivery and Customer Success

  • Act as the primary liaison for clients, optimizing application implementation and tailoring solutions to meet population health management needs.

  • Oversee vendor application deployments, troubleshooting, providing technical guidance, and delivering customized training for client teams.

  • Continuously engage clients to gather feedback, identify improvement opportunities, and ensure long-term success in care delivery.

  • Drive customer satisfaction through proactive support, issue resolution, and regular updates on application performance and features.

    Team Leadership and Development

  • Foster a collaborative, client-focused team culture dedicated to excellence, innovation, and continuous improvement.

  • Oversee resource planning and allocation to ensure timely project delivery and ongoing support for value-based care initiatives.

    Strategic Planning and Continuous Improvement

  • Develop and execute long-term strategies for vendor applications and data management, aligning with organizational objectives.

  • Identify and implement process improvements to enhance operational efficiency and patient experiences.

  • Stay updated on industry trends, emerging technologies, and best practices in value-based care to continuously improve applications and workflows.

    Stakeholder Engagement and Communication

  • Serve as the primary point of contact for vendor applications and data management, providing regular updates to leadership on performance metrics and opportunities.

  • Collaborate with cross-functional teams (e.g., IT, clinical services, sales) to align on client requirements, implementation timelines, and integration goals.

  • Create and deliver presentations and reports to inform stakeholders about initiatives, progress, and health outcomes.

    Qualifications

  • Education: Bachelor’s degree in Information Technology, Public Health, Healthcare Administration, Data Management, or a related field (Master’s preferred).

  • Experience: 8+ years in vendor management, application implementation, or data management, with 3+ years in leadership within healthcare or population health.

  • Proven experience managing end-to-end vendor application implementations and data integration projects in value-based care.

  • Strong understanding of data governance, quality standards, and healthcare privacy regulations (e.g., HIPAA).

  • Expertise in customer-focused project management, including application customization and troubleshooting.

  • Technical proficiency in data management platforms, integration tools, and cloud-based healthcare applications.

  • Excellent project management, communication, and interpersonal skills to work effectively with cross-functional teams and stakeholders.

  • Strategic mindset with strong analytical and problem-solving abilities.
