Licensed Practical Nurse (LPN) – Medical Respite Center
The Rescue Mission of Trenton seek s two License Practical Nurses (LPN) to work in our Medical Respite Center. The position will be responsible for supporting the daily operations of the Medical Respite Center, providing direct client care, and assisting clients in accessing necessary medical and social services. The LPN's will work under the supervision of the Program Manager/Registered Nurse and collaborate with a multidisciplinary team to ensure the health and well-being of clients.

This position is an important part of helping our clients accessing medical and social services to help ensure their health and well-being.
Position Details
Conduct initial client intake assessments
Perform routine health checks on clients and monitor and document any changes in their condition.
Provide direct nursing care as needed, following the care plan
Assist clients in scheduling follow-up medical appointments
Establish visits with client and families
Coordinate with external healthcare providers for comprehensive care
Provide referrals and information for community available medical and social services resources
Enter detail notes in Homeless Management Information System (HMIS)
Provide education sessions on the care process
Qualifications:
Graduate of an accredited LPN program.
Active LPN license; CPR certification required.
Minimum of 2 years of nursing experience, preferably in a community health, long-term care, or social services setting.
Proficient in basic computer applications and electronic health records

Ability to work flexible hours, including weekends and holidays
Shifts : Sunday - Thursday; 7a-3p Tuesday - Saturday; 3p-11p
Rate: $28.64 plus a comprehensive benefit and training package
