WHAT YOU WILL DO

The Home Therapies Dialysis Nurse, Expert will lead, mentor, and innovate as a SHC coordinator of patient care, collaborating with other care providers and health team members to provide required care. The RN 5 EXPERT acts as a patient teacher and advocate, assisting the patient in seeking information, assuring the patient has the opportunity for informed consent for treatment decisions, and promotes the maximum level of patient-desired independence.

The Home Therapies Dialysis Nurse, Expert has responsibility for coordinating the services related to home modalities including patient training, follow-up care, primary nursing and case management, education, and promotion of home treatment modalities to patients and staff.

Will perform home visits to determine that the home environment is suitable for patient dialyzing at home and whether or not the home will be a safe and appropriate setting in compliance with CMS regulations.

MINIMUM QUALIFICATIONS

Experience:

• Minimum of five (5) years of RN experience

• Minimum of five (5) years of experience as a Nephrology Nurse with dialysis experience required; in peritoneal dialysis, home hemodialysis, or a combination of both.

Education :

• Graduation from an accredited nursing school or equivalent; master's degree preferred

License/Certification :

• Current, active, unencumbered state RN licensure

• Current Healthcare Provider certification (CPR) required

• CNN/CDN Certification or within 12 months

Minimum Knowledge, Skills & Abilities :

• Competent and in-depth knowledge regarding the use of Electronic Health Records

• Ability to organize, prioritize and complete projects independently

• Valid state driver's license and reliable transportation for home visits

$42.14 - $57.54

The compensation for the role will depend on several factors, including the candidate's qualifications, work experience, competencies, and skills, and may fall outside of the range shown.
