Obran Cooperative Clinical Manager (RN) for Home Health in Torrance, California

Physician's Choice Home Health is seeking a Clinical Manager, RN, to join our Home Healthcare Team. In this role, you will leverage your experience in home health nursing to assist the Director of Nursing by planning, supporting field clinicians, and directing home health services. By joining our team, you will work alongside compassionate and dedicated healthcare professionals who are committed to delivering the highest level of care to our members.

The primary functions of this role are to assist the Director of Nursing with office tasks and provide support that requires a Registered Nurse (RN) license. You will ensure that patients receive care in accordance with the agency's policies, procedures, and best practices.

You will be involved in the following areas:

  • Handling incoming calls from patients and staff regarding clinical questions. If additional information is needed, you will escalate the call to the Director of Nursing and ensure a prompt response.

  • Overseeing the OASIS transfer documentation, following up on phone calls related to Hospital Hold status, and managing the Resumption of Care/Discharge process for patients admitted to the hospital.

  • Reviewing and signing Start of Care Orders, Recertification Orders, Resumption of Care orders, Discharge Orders, and external orders.

  • Reviewing and signing OASIS-Discharge and OASIS non-bill Discharges.

  • Managing High Intensity patients, including intake, scheduling, and meetings with Optum, among other responsibilities.

Requirements

Education: Graduate from an accredited diploma, associate or baccalaureate nursing program.

Licensure: Active, state license as a Registered Nurse (RN). Current driver’s license.

Experience: Home health experience with knowledge of intake, scheduling, authorizations, OASIS, and orders.

Skills: Proof of current CPR certification. Expert in back-office tasks. Ability to communicate with others regarding home health issues. QAPI / Chart Review Experience.

Transportation: Reliable transportation and valid auto liability insurance.

Rates and Salary Range Disclaimer:

  • The salary range for this position is $90,000-$100,000 per year.

  • The rates listed represent the low and high end of the range for this position. Visit and reimbursement rates vary based on visit type and employment status (1099 or W2). Rates may be adjusted in the future, and employees will be notified of these changes.
