Primary Function/General Purpose of Position

LCSW completes in-depth assessments of patients in order to evaluate candidacy for solid organ transplant. LCSW provides counseling, support, and community resource referrals to aide patients/families in preparing for transplant. LCSW follows patients pre- and post-transplant to assist with resolving psychosocial issues and lifestyle changes that improve a patient’s post-transplant success. LCSW works closely with multi-disciplinary transplant team to assess inpatients in order to identify appropriate discharge plans. LCSW provides assessment and intervention planning with patients experiencing substance abuse and/or mental health issues. LCSW will participate in community outreach and patient education related to transplant.

Minimum Qualifications:

Licensure/Certification/Registration: LCSW required.

Education: Master’s in Social Work from an accredited program required

Experience: At least three years post-Master’s experience preferred, preferably in a transplant setting.
