  Perinatal Navigators Essential Job Duties:

• SNF experience preferred

• Will work directly with post-acute NP to coordinate care of ACO and Medicare patients

• Will follow up on labs, follow up appointments, plan of care, and advanced directives

• Will also work with post-acute coordinate and social worker.

• Participate in nursing home huddles.

• Review discharges to identify those high risk for readmission and communicate findings to NP.

  Education, Training & Experience

• Must have active IN RN license

• Must have graduated from an accredited nursing program

• Must have active BLS certification
