    This is a per diem/part time position- Remote work plus one a day week onsite visit at several locations- 7 facilities ranging from Delhi / Norwich region, Albany region, CNY region and North Country SNFs.

    SNF regulation and experience required – to consult with the facilities social workers pertaining to day-to-day operations and regulations pertaining to social work processes.

    Salary range- $27-33 / hr rate- plus travel reimbursement for those 7 days a month.

    General Purpose:

    To assist the Director of Social Work Services in ensuring that residents' psycho-social interest are met and to assist in planning, organizing, implementing, evaluating and directing the overall operation of the Social Work Services Department in accordance with current Federal, State and Local standards governing the facility.

    Qualifications:

    • Must be registered with a Master of Social Work license, be in good standing and meet all applicable Federal and State guidelines for the provision of Social Services in New York State.

    • Must read, write, speak and understand English

    • Patience, tact, enthusiasm and positive attitude toward the elderly

    Essential Job Functions:

    A. Administrative Duties:

    Provide professional social services that conform to standards set by the Federal and State regulatory agencies for reasonableness and necessity of services, professional responsibility and quality assurance. Agrees to use their best efforts and skills to devote whatever time is reasonable and necessary to properly perform the services requested. Agrees to be available by telephone as needed to the Director of Social Work or designee. Provide written documentation of the services provided and make recommendations to the Director of Social Work. Notify the Facility of dates not available. Adhere to all facility policies and procedures and assist in the evaluation and development of policies and procedures. Be available Monday through Friday or other mutually agreed upon dates and times, as requested. It is understood that because you will be dealing with the confidential and proprietary information of both the residents and facility you agree that you will not disclose to anyone, directly or indirectly, any such information for use or derive any benefit whatsoever, directly or indirectly from such information, other than in the course of your performance.

    At the discretion and/or need of the Director of Social Work and/or Administrator assist in planning, developing, organizing, implementing, evaluating, supervising and directing the social services programs and activities; consult with and coordinate social services activities with administration, medical and nursing services, consultants and other related departments and assist in implementing changes as required; assist in determining plans for social work intervention, interpreting residents' social, psychological and emotional needs to the medical staff and other resident care team members; assist in coordinating the social services programs with the residents, their families, staff members and community agencies through interviews, consultations and direct involvement in programs; and, provide information to residents and their families, staff members and community agencies through interviews, consultations and direct involvement in the programs; and, provide information to the residents and their families concerning available support agencies and financial assistance programs to meet the needs of the residents.
