This position is a responsible professional position that reports to the Director of the Healthcare Quality and Clinical Management Unit for the Office of Children’s Medical Services Managed Care Plan. This position is responsible for promoting an integrated, comprehensive, and cost-effective health care system with a focus on health equity and social determinants of health to ensure access to quality care for CMS Plan enrollees. This incumbent will serve as the Enhanced Care Coordination (ECC) subject matter expert and will perform independent and complex work. This role is responsible for coordinating resources, leading implementing projects, and day-to-day coordination, oversight, and monitoring for efficient operation of Enhanced Care Coordination Program. This incumbent will be a Community Health Nursing Consultant for related CMS Plan Program initiatives and activities which include but are not limited to Care Coordination/Case Management and Enhanced Care Coordination.

Who We Are:

Florida Department of Health Children’s Medical Services (CMS) Managed Care Plan is responsible for the program oversight of the CMS Health Plan. CMS Health Plan provides benefits for children up to age 21 who have chronic and serious health condition(s)

Your Specific Responsibilities:

  • Providing on-going expertise, consultation, technical assistance and monitoring for the Enhanced Care Coordination components of the CMS Managed Care Plan.

  • Collecting, analyzing, and reporting data pertaining to an integrated system of care which includes Private duty nursing (PDN), Prescribed Pediatric Extended Care (PPEC), and skilled nursing facility (SNF) populations.

  • Serving as the subject matter expert and the Plan’s liaison with the Agency for Health Care Administration and other program vendors for all ECC programs and services.

  • Participating, coordinating, and ensuring CMS Plan vendor representation for all staffings, workgroups, and meetings related to ECC. Ensuring maintenance of a tracking log of staffings and providing consultation and technical assistance on follow ups needed with the CMS Plan vendor.

  • Performing programmatic clinical monitoring for the CMS Plan ECC populations including but not limited to enrollee record reviews, report reviews, program reviews, and contract reviews in a timely manner.

  • Maintaining electronic file with written instructions for each component of the job detailing who gets copied on emails, notifications, etc. File is updated if the instructions change and is reviewed quarterly for accuracy

  • Monitoring and leading initiatives for quality improvement projects in the area of enhanced care coordination.

  • Assisting in investigation and resolution of member quality of care complaints when escalated to the CMS Managed Care Plan Central Office for clinical reasons related to ECC and keeps detailed records of each.

  • Providing technical assistance to contract vendors to meet reporting requirements with defined timeframes for assigned projects regarding enhanced care coordination.

  • Tracking and monitoring disaster planning for CMS enrollees, including but not limited to, reviewing and tracking high risk population emergency preparedness plans and collaborating with internal and external stakeholders to ensure enrollees have access to appropriate emergency sheltering as needed.

  • Working intensively with vendors related to recommendations and best practices for the enhanced care coordination program.

  • Working closely with the contracted vendor to ensure the required ECC reports and any ad hoc reports are complete and accurate.

  • Tracking ECC enrollees transitioning and aging out of CMS within the year to ensure all protocols are followed and for making the CARES referral as applicable.

  • Performing collection, analysis and write up of evaluation of the enrollee record reviews and reports related to enhanced care coordination.

  • Analyzing QI data for reporting purposes looking for trends.

  • Overseeing and monitoring the development, revision, and implementation of the care coordination/case management programs.

  • Providing leadership for implementing changes targeted at systems and process improvements. Monitoring and evaluating the effectiveness of these improvement projects.

  • Participating in professional development trainings and programs to gain and maintain knowledge of ECC, care coordination/case management, and managed care for children and youth with special health care needs.

  • Providing comprehensive updates and generating reports and presentations when needed regarding the status of the enhanced care coordination program.

  • Performing program monitoring (desk and on-site when needed) to ensure that all requirements of the AHCA contract and the contracted vendor contract are being met in relation to the enhanced care coordination program.

  • Reviewing guidelines, statutes, and contract requirements related to enhanced care coordination to provide technical assistance to providers, vendors, and partners.

  • Ensure Nursing Facility enrollees have a completed and updated Individualized transition plan as required by AHCA guidelines. Ensure Individualized transition plans for all Nursing Facility enrollees reflect the transition planning process and any new circumstances, barriers, or considerations relevant to the enrollees transition to the home or community

    Performs other related duties as assigned including, but not limited to, training new staff, and assisting other unit members With assignments as needed, assisting with AdHoc report reviews, and reviewing member/provider material reviews.

    Assists the Healthcare Quality and Clinical Management Director with special projects as assigned.

    Required Knowledge, Skills, and Abilities:

  • Registered Nurse License required

    Qualifications:

  • This position is required to have and maintain a clear and active license as a Registered Nurse or be eligible to practice nursing in accordance with the Florida Administrative Code as defined by the Florida Board of Nursing.

  • Years of Experience: 1-3 Years of pediatric clinical experience preferred.
