ProMedica Health System Social Worker II - Medical Oncology - Full Time - Days in Sylvania, Ohio

Provides a wide array of social work services to oncology and infusion patients and families to promote understanding and resolution of problems related to oncology disease, environmental stress, physical illness, interpersonal conflicts, and other psychosocial issues. Assists patients and family by promoting coping skills, self-advocacy, sharing information and resource referrals.

ACCOUNTABILITIES All duties listed below are essential unless noted otherwise 1. Interviews patients, relatives and other to assess their personal and social adjustment and determine necessary social intervention. 2. Counsels patient/families to enhance their understanding and acceptance of personal and situational problems; helps them to change maladaptive patterns of behavior and make decisions about problems resulting from illness or disability. 3. Collaborates with multi-disciplinary team/other ProMedica cancer centers sharing social information for comprehensive medical and social care planning, referral to appropriate resources and community agencies for counseling, home health care, rehabilitation, treatment, and discharge planning including supplies and insurance concerns for patients and families. 4. Facilitate patient referrals with needed services as appropriate including needed supplies and insurance concerns. 5. Implements social work oncology support groups as needed and engages in formal and informal education of patient, families, medical and nursing staff as needed. 6. Documents patient/family care and progress in the electronic medical record. 7. Participates in team conferences and education. 8. Performs other duties as directed. Education: Master’s degree in Social Work Skills: Experience in a medical or other multi-disciplinary agency. Years of Experience: 1-2 years License: LSW in the state of Ohio Certification: N/A

PREFERRED QUALIFICATIONS Education: N/A Skills: Prior experience in health care, behavioral health setting or community agency. Years of Experience: 2+ years in multi-disciplinary agency License: LISW in the state of Ohio; LMSW in state of Michigan Certification: N/A

ADDITIONAL EXPERIENCE Must be able to understand directions and be able to communicate and respond to inquiries; requires effective interpersonal skills and customer service skills. Exceptional problem-solving, critical thinking, organizational, interpersonal, and written/verbal communication skills. Ability to work in self-directed environment with attention to detail, follow through, and has high level of computer skills.

WORKING CONDITIONS Personal Protective Equipment: Must be able to tolerate use of contact precautions. Physical Demands: Must be able to move about the office between workstations.
