Job Description/Essential Functions:

Horizon House, Inc. is a nationally recognized provider of services to adults with support needs due to developmental disabilities, behavioral health issues, and substance abuse. Horizon House is Pennsylvania's oldest psychiatric rehabilitation agency. Since 1952, we have been dedicated to helping people with mental illness, drug and alcohol addictions, developmental/intellectual disabilities, and homelessness to stabilize their lives and to help them live as independently as possible in the community. With over 100 locations throughout South and North Eastern Pennsylvania and the state of Delaware, Horizon House provides services to a diverse population focusing on individual strength and choice.

Our Suburban Behavioral Health Services Division is seeking a dedicated Behavior Health Specialist to fill the following Full time opening. This position is based in our Swarthmore, Delaware County office. The Behavior Health Specialist is responsible to conduct initial client intake and assessment for individuals referred for treatment; to provide outreach and case management services to stabilize homeless clients with serious mental illness and substance dependence; to promote self-sufficiency and independent living for client caseload; to transfer clients to other forms of care as needed; to comply with funding source regulations and agency policies and procedures; to provide on-going review and assessment of client services and growth, within established system' and to ensure continuing of services in compliance with funding sources and agency Policy and Procedures.

This position is responsible for the following duties:

To conduct initial client intake and assessment for individuals  referred for treatment.

To provide outreach and case management services to stabilize homeless clients with serious mental illness and substance dependence.

To promote self-sufficiency and independent living for client case load.

To transfer clients to other forms of care as needed.

To comply with funding source regulations and agency policy and procedures.

To provide on going review and assessment of client services and growth within the established system.

To ensure continuing  of services in compliance with funding sources and agency policies and procedures.

Job Qualifications:

Bachelors degree in Human Services, Social Sciences, Healthcare or related field, and at least 3 years experience in Mental Health.

Knowledge of Mental Health systems and supports.

Excellent communication skills both oral and written.

Detailed orientated.

Excellent assessment skills with at least 1 year case management and counseling experience.

Possess a valid drivers license with an acceptable driving record.

Scope:

Requires the ability to lift, carry, fold, kneel, stack, stoop and bend which may include direct intervention with participants.

Possess manual dexterity and fine motor skills.

Must be available for local travel.

May require crisis intervention to emergency situations.

May be required to carry agency cell phone during scheduled emergency on-call rotation.
