As a Registered Nurse (RN) Case Manager, you will work collaboratively with inter-professional teams across the continuum of care to facilitate and ensure effective transitional care coordination. Essential to this role are the core values of patient advocacy, holistic assessment, care planning and the empowerment of patients and families to self-manage. Case Managers also ensure compliance with the standards of the accrediting bodies, state mandated revised code and the CMS Conditions of Participation.

The hours for this per diem RN Case Manager Opportunity is 8AM-4:30PM. Weekend coverage will be required. 2 weekend shifts in a 6-week period.

Required qualifications for the ideal candidate include:

• Education - Graduate from an accredited school of professional nursing.

• Have current clear and active Florida RN License

• Certification by an approved entity preferred within one year of eligibility, e.g. CCTM, ACM, CCM. American Heart Association.

• American Heart Associate Basic Life Support for Healthcare provider

• Minimum of three years of recent RN clinical experience

• Empathy

• Compassion

• Obtain BSN within first 3 yrs. of employment
