Stony Brook Medicine (Stony Brook University Hospital) Per Diem TH Social Worker II in Stony Brook, New York

Position Summary:

At the Long Island State Veterans Home, the Per diem TH Social Worker II is an integral part of our interdisciplinary health care team. The incumbent provides the most comprehensive service possible to be able to assess and identify psychosocial needs and develop criteria to assure that patients and families in need of social work assistance obtain needed services.

Duties of a Per Diem TH Social Worker II may include the following but are not limited to:

  • Provide Social Work Services to residents as assigned, including long term treatment, planned short term treatment or crisis intervention according to individual needs.
  • Assist resident family members and/or resident advocates in mediating facility and community systems.
  • Participate/initiate interdisciplinary team meetings for resident care provision and discharge planning.
  • Engage residents and/or families in professional treatment relationships to assess the need for Social Work intervention.
  • Provide social work practice to assist resident and/or family to cope with the psychosocial issues connected with illness and or end of life.
  • Maintain required documentation as per department policies and procedures and in compliance with regulatory requirements.

Working conditions may involve exposure to infectious disease, blood, body fluids and/or tissue as well as other unpleasant elements such as accidents, injuries and illness. May be exposed to and be required to handle varying unpredictable, emergency and crisis situations. Occasionally subjected to irregular hours and pressure due to multiple calls and inquiries.

Qualifications

Required Qualifications:

Master's Degree in Social Work from an accredited school of Social Work. NYS Social Work license. One year experience in a skilled nursing facility.

Preferred Qualifications:

Experience working with MDS, discharge planning, electronic medical records, and ADL software. Working experience with the Veteran population. LCSW and Supervision in Field Instruction (SIFI).

Essential Position: This position has been designated as an essential position based on the duties of the job and the functions performed. This means that when the Long Island State Veterans Home is faced with an emergency, employees in such positions are required to remain at their work location or to report to work.

The LISVH is a 24/7 skilled nursing facility. As such, all positions are subject to changes in pass days and shifts as necessary to meet the operating needs of the facility.

The selected candidate must successfully clear a background investigation. Prior to start date, the selected candidate must meet the following requirements: Successfully complete pre-employment physical examination and obtain medical clearance from the Long Island State Veterans Home Employee Health Department, clear a background investigation, submit three (3) written references, and provide a copy of the required license(s)/certificate(s). Please be advised that failure to comply with any of the above requirements could result in a delayed start date and/or revocation of the employment offer.
