Canvas Health is a non-profit, Certified Community Behavioral Health Clinic (CCBHC) specializing in serving clients who are on Medicaid. Position Summary Coordinates social services provided to residents of assigned senior and low-income buildings. Services include information and referral, outreach services, health and wellness education and assessment of service needs. Location Stillwater, MN Schedule Part-time, 24 hours/week with daytime hours varying days Monday through Friday Minimum Qualifications High school diploma or equivalent Thirty-six (36) hours of in age-related and disability issues training during the first year of employment Knowledge of the methods, values and principles of social work Knowledge of care management issues with seniors and persons with disabilities Ability to establish and maintain effective working relationships with others including Washington County staff, building managers and other professionals Ability to utilize counseling techniques with seniors and their families to achieve care plan goals Knowledge of pertinent state laws regarding vulnerable adult and some state housing regulations Knowledge of HIPAA regulations Ability to gather data, write reports and maintain clients' files accurately and up to date Working knowledge of computers and computer applications Ability to do public speaking and present information to residents, managers, stakeholders including funders and other housing and social service officials Ability to effectively and positively communicate and function as a team member and ability to work independently Preferred Qualifications Bachelor's degree in social work, Gerontology, Psychology or Counseling preferred; other college degree or comparable work experience may be considered 3 years clinical experience in community-based counseling Two (2) to three (3) years' experience in social service delivery with senior citizens and/or nonelderly disabled Experience coordinating training/educational opportunities and knowledge of referral resources As an essential healthcare provider, Canvas Health follows all local, state, and federal guidelines regarding COVID-19 precautions.
