Partners For Youth Case Manager - Community Based Mentoring in Steamboat Springs, Colorado

Our Community-Based Mentoring Program pairs youth (mentees) with selected and trained adult mentors. Each mentee is matched with a volunteer mentor who has been thoroughly recruited, screened, trained, and supported by a Partners Case Manager to ensure a positive and impactful relationship.

This is a 1.0 full time equivalent, non-exempt position. Starting salary Range $48K-$53K DOE. Benefits include health insurance, retirement, unlimited PTO, 11 paid holidays, Winter Holiday Week office closure, wellness benefit.

Required Qualifications: *Excellent verbal and written communication skills, ability to relate effectively with people of varying life experiences *Experience and/or passion for serving youth and families *Ability to maintain confidentiality *Ability to demonstrate strong problem solving skills *Ability to foster positive, supportive relationships with collaborative partners *Must be willing to work occasional evenings and weekends *Successful completion of Partners' screening process *Must have privately owned vehicle, current driver's license and up to date insurance, as well as a willingness to drive a 12-passenger van as needed in varying road conditions

Job Functions & Responsibilities Case Management: *Conduct interviewing, screening, training of volunteer adult mentor applicants *Conduct interviewing and perform intake procedures on referred mentees *Decide, with input from other staff, on appropriate matches of mentees and mentors & perform match meetings *Build and maintain healthy relationships with a caseload of matches, unmatched mentees and pending mentors, parents/guardians, and referral agencies *Keep consistent and adequate case notes and records on all referred mentees, mentors and matches *Support implementation of youth program evaluations *Maintain healthy relationships with and contact referral agencies to report on status and progress of referred and matched youth *Develop and maintain a working knowledge of community resources, agencies, and their function Program Support with fellow Case Manager: *Plan and implement program activities for matched and waiting youth *Implement mentor recruitment including but not limited to marketing, outreach, social media, in person events, etc. *Implement communication for mentees, mentors and parents/guardians
