About the Team The palliative care team within Lakewood Health System's Care Management team is a dedicated, small but mighty group focused on providing comprehensive, compassionate support. This multidisciplinary team includes clinical experts, social workers, and outreach specialists. They work collaboratively to address the physical, emotional, and social needs of patients and their families, ensuring patient-centered care. Their integrated approach bridges clinical care with community resources, facilitating seamless coordination and continuous support throughout each patient's journey Mission for Position To provide comprehensive, compassionate, and patient-centered support by coordinating clinical, social, and community resources, ensuring seamless care and enhanced quality of life for patients and their families. Duties & Responsibilities Coordinate palliative care services to meet patients' clinical, social, and emotional needs. Collaborate with a multidisciplinary team, including social workers and outreach specialists, for comprehensive patient support. Serve as a liaison between patients, families, healthcare providers, and community resources. Develop and implement individualized care plans to enhance patients' quality of life. Conduct assessments and provide education to patients and families on palliative care options and resources. Facilitate smooth transitions and continuity of care across healthcare settings. Position Type .8 FTE, Day Shifts M-F flexible scheduling opportunities. Qualifications Qualifications Experience in palliative care, care management, or related healthcare field. Strong knowledge of clinical and community resources. Interpersonal and communication skills for patient-centered care. Ability to work collaboratively in a team. Patient-focused approach with commitment to enhancing quality of life. Self-motivated with strong organizational skills. Ideal Candidate Strong Clinical and Assessment Skills: Proficiency in conducting thorough patient assessments and anticipating needs, ensuring effective patient monitoring and timely interventions. Empathy: Demonstrated ability to advocate for patients, fostering their involvement in care decisions, with a compassionate, patient-first approach. Collaborative Teamwork: Exceptional interpersonal skills for effective collaboration with interdisciplinary teams, patients, and families, contributing to a cohesive care environment. Effective Communication: Strong verbal and written communication skills, particularly in patient education using methods like Teach-Back to confirm comprehension. Adaptability and Change Management: A positive and open-minded approach to change, capable of supporting and driving improvement initiatives.
