The Urgent Response Services team is comprised of innovative, resourceful, and passionate clinicians and leverages their skills and competencies to bring urgent clinical services to our members - keeping them safe at home. This position requires deep understanding of what high value care means and what is required to deliver it inside the trusted relationship between clinicians, clients, families, and staff. High clinical acumen in geriatrics, strong listening skills, a desire to mentor, high level of flexibility and robust organizational skills are required strengths. Position Functions and Responsibilities This is additional duties as assigned to a full-time position assigned between clinical primary care management and Lead accountabilities. (allocations may be adapted to the needs of the company). Lead Accountabilities: In partnership with leader, lead onsite and virtual support and mentorship Assists with implementation and supervision of core processes and operational procedures to maximize quadruple aim success. Works in partnership with leader to implement and monitor practice standards. Perform service recovery as needed. Assist with tracking and monitoring the use of evidence based practice guidelines, assessment tools, standards, processes, and other tools to deliver best in class geriatric primary care as defined by our clinical leadership team. Assist in providing onboarding, training and coordinate field clinical orientation for staff. Participate and support IDT meetings to ensure team follows up on interventions, consults and changes in treatment plans. Set realistic expectations, including limitations. Builds accountability and monitors performance of clinical practice for team. Create a culture for change; inform quality and process improvement initiatives. Support and facilitate implementation of new initiatives. Manage/participate in special projects as assigned. Collaborate to identify medical necessity/appropriateness of ancillary services and interface directly with the providers and staff. Develop/strengthen relationships with both internal and external partners. Identify problems and develop solutions in collaboration with key team members. Qualifications Education: LPN, RN, Paramedic or equivalent Bachelor's degree (BS or BA) in healthcare or related field preferred. Experience: Minimum of 5 years of experience in a clinical setting. Previous experience in leadership roles preferred. Experience working with seniors preferred Experience in ustomer service/direct member contact field preferred Experience in multiple settings and situations Experience in long term goal setting and pro-active intervention Comfortable with complexity. Strong knowledge of medical management and commitment to use of evidence when it exists and acknowledgement when it does not.
