Hospice RN Case Manager
Schedule: Monday – Friday, 8:00 AM – 5:00 PM
Service Area: Saint Johns to Mt Pleasant (North Territory)

Your Impact
As a Hospice RN Case Manager , you will ensure comfort and compassion guide every interaction. Your responsibilities include:
Holistic Patient Care: Perform physical, emotional, and environmental assessments to create individualized care plans.
Meaningful Visits: Deliver hands-on nursing care, ensuring every interaction supports the patient’s well-being.
Team Collaboration: Communicate vital updates to supervisors, physicians, and care team members to align care strategies.
Empowering Families: Educate and support loved ones, offering guidance and resources to navigate their journey.
Proactive Response: Monitor and address changes in condition promptly, ensuring the patient’s care needs are always met.
Commitment to Excellence: Follow all care plans and physician directives, delivering care that prioritizes dignity and quality.
What You’ll Need
Graduation from an accredited nursing program (Certificate, Diploma, Associate, or Bachelor’s).
Current RN license in Michigan.
A minimum of 1 year of clinical nursing experience (hospice or home health preferred).
Reliable transportation for patient visits.
A dedication to multitasking, documentation, and meeting deadlines while delivering outstanding care.
