Springville. Jennie B. Richmond Nursing Home. LICENSED PRACTICAL NURSE (LPN). FT. Benefits. 1st and 3rd Shifts. $26.92- $29.54/hr.

Performs tasks and responsibilities within the framework of case finding, health teaching, health counseling and provision of supportive and restorative care in keeping with nursing care plan under the direction of a registered professional nurse. Does related work as required.

ESSENTIAL DUTIES AND RESPONSIBILITIES include the following. Some of the specific duties performed may vary by shift, depending on the individuals' particular assignment on that day. Other duties may be assigned as necessary.

Assists Health Care Team with the collection of data that can be used to complete the Patient Assessment Instrument, Minimum Data Set (MDS+) Resident Assessment Protocol Summary.

Assists Health Care Team with recording and interpreting resident responses to their plan of care.

Guides, supports, monitor and assists allied nursing personnel in care given to residents.

Monitors and upholds the allied nursing personnel to perform care within the standard of practice guidelines.

Maintains open lines of communication with all staff, residents and families.

Promotes safety standards throughout the facility (i.e. fire and safety).

Assists with orientation and mentoring of new personnel.

Participates in solving problems and making improvements on the units in collaboration with Nursing Management.

Participates in in-service education programs, attends all mandated annual programs.

Participates in the collecting of data for the Patient Review Instrument (PRI). Carries out all duties in an ethical and honest manner.

Performs according to Standards of Performance.

Understands and complies with all legal and other requirements in the performance of duties.

Reports willful or inadvertent potentially fraudulent activity to management or the Corporate Compliance Officer.

Completes hands on care of patients/residents as assigned within their scope of practice.

Understands and follows all facility policies and procedures as outlined.

Completes other assigned duties within the scope of their practice.

QUALIFICATIONS: To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. EDUCATION and/or EXPERIENCE: Received education and a diploma or degree in Professional Nursing, in accordance with the commissioner's regulations, and in order to continue to maintain registration as a Licensed Professional Nurse (LPN) in New York State.

CERTIFICATES, LICENSES, REGISTRATIONS: Current and unrestricted licensure as a Licensed Professional Nurse (LPN) in the State of New York.

Basic Life Support (BLS) certification required.
