South Middlesex Opportunity Council Case Manager, Young Parent Living Program in Springfield, Massachusetts

SUMMARY

The Open Pantry Teen Parenting Program is a residential program for single young parents age 14-22 and their child(ren). The Case Manager assists families by developing and maintaining a program, which includes but is not limited to: individual assessments and service plans, educational input and group interactions, as well as enrichment and educational programs.

PRIMARY RESPONSIBILITIES

  • Assist families in accessing services, setting goals and developing a plan for attaining those goals.

  • Assist in mediating difficulties between families/teens.

  • Model good parenting skills in both structured and informal ways.

  • Adhere to program procedures

  • Network with outside agencies and resources for long-term services as needed (daycare, schooling, healthcare, counseling, etc.)

  • Maintain accurate records for each family.

  • Engage in self-evaluation to develop professional goals.

  • Participate in maintaining a clean and safe atmosphere in the program.

  • Maintain valid drivers license in order to provide transportation for clients.

  • Perform family intakes

  • Participate in on-call responsibilities.

  • Strengthen working relationships with significant resources in client network.

  • Attend staff meetings and facilitates house meetings.

  • Attend agency required trainings.

  • Engage all clients by understanding and addressing their needs whether within or outside the scope of the specialty.

  • Attend & participate in engagement team meetings, follow up with client service plans and communicate effectively with clients and staff in other specialty areas.

  • Ensure compliance with program/department, agency and/or funder requirements, as well as SMOC policies & procedures.

  • Other duties as assigned.

KNOWLEDGE AND SKILL REQUIREMENTS

  • Previous experience working with homeless and/or teens preferred.

  • Strong verbal and written communication skills.

  • Bi-lingual preferred.

  • Bachelor’s degree in counseling, social work, psychology or a related field required or trained as a paraprofessional with at least three years of experience.

  • Must have a valid driver’s license.

ORGANIZATIONAL RELATIONSHIP

  • Directly reports to Program Director.

  • Direct reports of this position are NONE.

WORKING CONDITIONS

The program is operated out of a three-story house located in the North end of Springfield, MA. Staff are required to do hourly house checks and be able to respond to guests needs on all floors. There is minimal lifting involved. Driving is also a requirement as the program is required to pick up all referrals to the program. As part of the responsibilities of this position, the Case Manager will have direct or incidental contact with clients served by SMOC in various programs funded or administered through the Executive Office of Health and Human Services. Must pass a background check
