Job Title: Afterhours Hospice Registered Nurse (RN)

Schedule: 7 days on/7 days off

Your Role as an Afterhours RN:

As an Afterhours RN, you'll be at the forefront of patient care, providing crucial support and nursing expertise outside of regular hours. You will:

  • Patient Assessment: Evaluate assigned home health care patients to identify physical, psychosocial, and environmental needs, ensuring that each visit aligns with the patient's care plan.

  • Initial & Ongoing Evaluation: Conduct initial evaluations and continuously assess nursing needs, adapting care plans as necessary to meet changing patient conditions.

  • Effective Communication: Relay significant findings, changes in health status, or environmental concerns to clinical supervisors, physicians, and other care team members.

  • Patient & Family Support: Offer counseling, instruction, and guidance on patient safety, leveraging family and community resources as needed.

  • Care Plan Implementation: Execute the plan of care for an assigned caseload, ensuring quality, continuity, and achievement of desired patient outcomes.

  • Nursing Standards: Identify deviations from standard patient conditions and take appropriate action to address these changes.

  • Plan Verification: Confirm care plans before each visit and provide care following physician’s orders, assessment data, and established standards.

Qualifications & Requirements:

  • Nursing Education: Graduate of an accredited Certificate, Diploma, Associates, or Baccalaureate School of Nursing.

  • Clinical Experience: At least 1 year of experience as a nurse in a clinical care setting; home health experience is preferred but not mandatory.

  • Licensure: Current State license as a Registered Nurse.

  • Travel: Ability to travel within a 50% radius of your coverage area.

  • Communication Skills: Strong verbal and written communication abilities, with a focus on patient-centered care.
