Summary: Provide Supportive Services to Veterans and their families in this grant funded program. Provide case management services to Veterans to develop and execute Individual Housing Plans that enhance housing security to eligible participants through direct and indirect service provision in Goodwill and partner agencies. Must have valid driver license with personal auto insurance as well as pass background and driver record check.

Education and/or Experience:

Bachelor's degree from four-year college or university; or one to two years related experience and/or training; or equivalent combination of education and experience.

Essential Duties and Responsibilities include the following:

Effectively assist participants to achieve and maintain housing stability by coordinating and/or providing supportive services.

Ensure that participants receive the assistance needed to promote housing stability, including but not limited to: consulting with participants to conduct needs assessment, working with participants to create individualized housing stability plans that include specific plans and specific goals, monitoring participants from the time of intake until they exit the program, and ensuring that participants obtain needed supportive services, re-evaluate participant eligibility, per program requirements.
