Providence St. Joseph Health Clinical Pharmacy Specialist - Primary Care (0.8 FTE) in Spokane, Washington

Description The Clinical Pharmacist Specialist will collaborate with physicians and staff to provide coordinated, comprehensive, high quality care to patients. The responsibilities of the clinical pharmacist will predominantly fall into the following areas: (1) Patient Care - The Clinical Pharmacist Specialist is responsible for providing collaborative practice services in collaboration with the healthcare team. Required qualifications: Bachelor's Degree in PHARMACY from an accredited School of Pharmacy. Or Upon hire: Washington Pharmacist License Within 2 years of hire: Board Certified by the Board of Pharmacy Specialties specific to area of practice within 2 years of starting the role Upon hire: Pharmacy preceptor license in applicable state as applicable Two years of Senior Ambulatory Clinical Pharmacist experience within relevant practice setting - OR - FIVE years of pharmacy practice experience with at least 2 years of experience in relevant setting in lieu of PGY-1 and PGY-2 residency requirement. Preferred qualifications: Coursework/Training and Postgraduate Year One (PGY-1) AND Year Two (PGY-2) Pharmacy Residency or equivalent experience Or equivalent educ/experience Why Join Providence?
