LifePoint Health Patient Navigator-Lung Nodule in Somerset, Kentucky

The Patient Navigator functions as a part of a multidisciplinary team as an advocate, interpreter, educator, and counselor for patients with lung nodules. This position will be responsible for development and coordination of this program. The Navigator will be responsible for ensuring all patients with a lung nodule diagnosis receive quality and comprehensive services, coordinating patient care throughout the continuum in collaboration with the multidisciplinary team, and serving as a clinical resource with expertise in lung nodule management. This position will serve as a liaison throughout the facility and in the community regarding services provided for this unique patient population through clinical knowledge expertise, consultation, education, and research. In the execution of job duties, it is the universal expectation that all tasks are performed with a patient centered focus, while also seeking opportunities to continually improve core processes. Minimum Education• Associate's degree □ Preferred X Required• Bachelor's degree X Preferred □ Required

Required LicensesRegistered Nurse Current Registered Nurse Licensure to practice in the state of Kentucky OR a Current Registered Nurse Licensure with a multistate compact status or Registered Respiratory Therapist licensed to practice in the state of Kentucky.Oncology Nursing Certification preferred

Minimum Work Experience• Preferred experience in clinical oncology environment.• Preferred experience in case management or similar navigator role.

Required SkillsCertifications:1. Current American Heart Association BLS Certification (or) obtained within 90 days after hire date. Requires critical thinking skills, decisive judgment and the ability to work with minimal supervision. Must be able to work in a stressful environment and take appropriate action. Language Skills: Able to communicate effectively in English, both verbally and in writing.
