Key Responsibilities:

  • Comprehensive Care Management: Assess, plan, implement, and evaluate direct care for adult day care participants, ensuring each individual's plan of care is meticulously developed and regularly reviewed.

  • Collaborative Care: Work closely with physicians, medical professionals, and adult day care staff to maintain high-quality care as outlined in the care plan.

  • Holistic Approach: Utilize your knowledge of growth and development principles to meet the unique physical and psychosocial needs of participants, involving caregivers, physicians, and external organizations as needed.

  • Leadership and Administration: Partner with the center director to oversee nursing and medical needs, assess current activities, and implement effective programs and practices.

  • Regulatory Compliance: Ensure all nursing care and medical services comply with State and Medicaid regulations, including timely documentation.

Minimum Knowledge, Skills, & Abilities Required:

Minimum five (5) years related clinical experience required. Minimum of three (3) years management experience in comparable area of an acute care setting is required.

Candidate must be capable of:

  • Assessing current unit activities/problems in a clinical setting, evaluating alternative approaches, implementing programs/practices, and evaluating outcomes.

  • Maintaining effective relationships with peers, other departmental staff and physicians.

  • Effectively managing human, material, capital, and financial resources
