Community Clinic Inc. Nurse Manager in Silver Spring, Maryland

  • Has 7 health center sites in underserved communities throughout Prince George’s and Montgomery Counties serving over 34,000 patients.

  • Oversees several federal, state, and county contracts and/or grants that focus on clinical operations, patient outcomes, and access to care and services.

  • Provides health assessments for refugees, asylees, and special visa holders for the state of Maryland.  The program currently sees around 2,100 patients on an annual basis.

POSTION SUMMARY: 

The Nurse Manager under the direction of Director of Nursing and the clinical care team, serves as an integral member of the nursing and CCI health center leadership teams.  As a member of the health center triad, the Nurse Manager participates in administrative and organizational leadership activities to promote efficiency and optimal quality of care.  They are responsible for assisting and supporting the CCI team in providing high quality, direct, evidence-based care and care managing high risk patients with chronic illnesses, to promote health education, condition management and self-advocacy to ensure improved patient and financial outcomes as well as work to achieve annual clinical metrics.  

 

KEY FUNCTIONS AND RESPONSIBILITIES:

  • Supervises and leads the nursing and medical assistant teams including staffing, hiring, onboarding, training, annual evaluation, and necessary corrective action. 

  • Manages daily clinical operations to assure optimal efficiency, clinical safety and high-quality care. 

  • Fosters communication and collaboration among members of the care team. 

  • Oversees in-house laboratory operations and continuous review of sterilization equipment and in-house lab test’s quality control and maintenance logs. 

  • Leads clinical huddles, transitions of care and pre-visit planning efforts.  

  • Develops a working rapport with patients through nursing co-visits, care coordination and management duties for the entire lifespan of CCI’s patient population. 

  • Collaborates with clinician team to identify complex care patients.   

  • Partners with care team to develop care plans, set patient-centered goals and continually assess patient’s self-advocacy, self-management, and barriers to care.  

  • Provides appropriate health education and discharge instructions with the support of evidence-based resources. 

  • Provides verbal and written information in the patient’s preferred language by utilizing in-person, phone, or virtual interpreters. 

  • Leads initiatives to promote and improve quality, safety and patient satisfaction to meet CCI quality goals. 

  • Provides regular clinical coverage for clinical support positions agency wide. 

  • Collaborates with practice managers on inventory management and ordering of supplies, pharmaceuticals, and durable medical equipment. 

  • Plans, coordinates, and/or attends assigned meetings, health fairs, events, and trainings. 

  • Consistently seeks avenues for self-education to broaden knowledge base and maintain compliance with evidence-based practice.  

  • Uses the Electronic Medical Record (EMR) to maintain patient medical records, utilizing standard language and abbreviations and following accepted medical standards for charting. 

  • Provides quality customer service and communicates in a HIPAA compliant, culturally sensitive, and trauma-informed manner.  

  • Implements new and existing CCI Health Services clinical guidelines, workflows, policies and procedures, and ensures clinical staff adherence to Joint Commission, HIPAA, PCMH, performance improvement, and compliance policies. 

  • Performs other duties determined as necessary by the Director of Nursing and other CCI leadership. 

MINIMUM QUALIFICATIONS: 

  • Active RN license issued by the State of Maryland Board of Nursing or any state under the Nurse License Compact, B.S. in nursing preferred.  

  • Minimum of three (3) years of nursing experience demonstrating progressive leadership experience and experience in a supervisory capacity. 

  • Current BLS/CPR certification required. 

  • Experience with documenting in an Electronic Medical Record (EMR) – specific knowledge of eClinicalWorks is a plus. 

  • Written and oral fluency in English, bilingual preferred.
