Location: Shelton Office

Schedule: Monday - Friday, 8:00 AM - 5:00 PM

What You’ll Do:

As the Behavioral Health Clinical Supervisor (RN) , you’ll be the cornerstone of patient care and leadership. Your role includes:

  • Lead with Impact: Oversee the delivery of high-quality, specialized behavioral health services, ensuring patients receive exceptional, tailored care at home.

  • Coordinate Care Teams: Collaborate with healthcare providers, families, and external partners to ensure seamless communication and patient care coordination.

  • Foster Excellence: Ensure regulatory compliance and maintain high standards for quality care by supervising timely care plan implementation and ongoing communication.

  • Monitor & Adapt: Assess patient needs regularly, approve customized care plans, and make necessary adjustments to optimize outcomes.

  • Drive Quality: Champion a patient-first approach by monitoring every stage of care, ensuring compassionate and effective service at all times.

What You Bring:

  • Bachelor’s degree preferred but not required.

  • 3+ years of experience as an RN , w ith at least 1 year in home care is required.

  • Deep knowledge of Medicare/Medicaid home care benefits, policies, and procedures.

  • Familiarity with Homecare HomeBase (HCHB) is a big plus.

  • Valid driver’s license and insurance , along with reliable transportation.
