Professional Staff Nurse

Transitional Care Unit (TCU)

Casual status (36 hours each 6-week schedule)

The unit is a unique department on a hospital campus, bridging the gap between acute care and home. We help patients regain strength and independence through therapy and skilled care, ensuring better outcomes. With a collaborative, multidisciplinary approach, we provide comprehensive, patient-centered support for a smooth recovery.

Responsibilities:

  • Provides direct patient care as a Registered Nurse, ensuring safe, high-quality, and compassionate treatment.

  • Applies adult learning principles to educate patients, families, students, and new staff.

  • Guides patients and families through care transitions with detailed education.

  • Supports staff development, serves as a preceptor, and fosters open dialogue for growth.

  • Engages in professional development and shared governance to enhance patient care.

  • Actively participates in improving the practice environment and adapting to change.

  • Builds strong relationships, demonstrates cultural awareness, and promotes work-life balance.

  • Uses the nursing process to develop holistic, patient-centered care plans.

  • Communicates effectively with care teams, ensuring accurate documentation and patient advocacy.

  • Participates in quality improvement initiatives and promotes evidence-based practice.

  • Takes responsibility for improving patient satisfaction and overall care quality

  • Demonstrates knowledge of age-specific patient care and growth/development principles.

  • Minimum 1 year of experience required

  • Prior long term care experience preferred

  • Ability to build positive relationships with staff, physicians, and patients/families.

  • Strong clinical judgment, critical thinking, and problem-solving skills required.

  • Requires mobility, dexterity, and physical stamina for patient care tasks. Licensure, Certifications, and Clearances:

  • Basic Life Support (BLS) OR Cardiopulmonary Resuscitation (CPR)

  • Registered Nurse (RN)

  • Act 34

  • OAPSA

*Current licensure either in the state where the facility is located or, if the facility is in a state covered by the multistate Nursing Licensure Compact (NLC) agreement, a multistate license issued by a participating NLC state. Hires and current employees working on an out-of-state NLC license who later change their residency to the state where the facility is also located will have 60 days upon changing their residency to apply for licensure within that state.
