Job Summary The Social Health Portfolio Manager is responsible for the oversight and continuous improvement of a regional Social Health Portfolio and initiatives. The position acts as an innovator and integrator across the network, convening Coordinated Care Organization (CCO) partners to co-create, learn together, and spread emerging best practices with the goal of improving social health needs for CareOregon members. This highly creative leadership position is responsible for promoting a great degree of intra-departmental collaboration, ensuring strategic and operational integration, and incorporating subject matter expertise as needed. Essential Responsibilities Participate in the development, implementation, and evaluation of social health programs around Traditional Health Workers, Health Related Services, Health Related Social Needs, and Community Information Exchange, and other social health initiatives in partnership with CareOregon stakeholders and provider network inclusive of Community-based Organizations. Create, recommend, facilitate, and evaluate social health strategies, tools, and processes to drive efficiency and improve health outcomes among CareOregon's regional populations. Serve as the content expert for social health best practices for the region. Support collaboration with delivery system leaders across the region to build a strong foundation of social health integration. Support strategic partnerships with community-based organizations and social service organizations to improve access to quality social health services for members. Support collaborative relationships with providers that promote and achieve Quintuple Aim goals. Maintain appropriate records, information, trackers, documentation, and productivity reports during and at the time of project/program completion. Ensure strategic performance goals, expectations and standards are clearly understood by stakeholders. Evaluate the Social Health Portfolio performance on an ongoing basis and take appropriate corrective action. Develop communication materials (slide presentations, reports, etc.) to educate and report on social health projects, programs, and initiatives. Represent CareOregon on appropriate local, state, and national committees and work groups. Strategically inform and provide guidance from a social health perspective to CareOregon teams.
