UW Medical Center has an outstanding opportunity for a RN Manager to join our Nurse Care Coordination team. WORK SCHEDULE

  • Full Time, Days

  • Monday-Friday DEPARTMENT DESCRIPTION Our Care Management team's goal is to achieve coordinated patient flow through an inpatient stay, culminating in a safe and timely discharge, and work closely with all members of the interdisciplinary health care team, ensuring that the team is aware of the patient’s plan of care and that all disciplines are working toward the same goals for the patient. PRIMARY JOB RESPONSIBILITIES

  • Oversees the staff and processes that support patient progression of care throughout the continuum, including: emergency services, pre-admission, inpatient stay, post-acute care strategies and intensive outpatient case management.

  • Provides leadership and vision to positively help build collaborative and flexible teams with clear goals.

  • Provides oversight to staff who ensure all incoming patients are screened to identify potential discharge barriersand readmission risks, and that all service teams are moving the patient forward to discharge, eliminating orlimiting internal or external barriers throughout the stay.

  • Oversees, or collaborates with partners/departments to track LOS, Discharge Delays and Readmission metrics and helps to identify best practices in progression of care, discharge planning and transitions of care.

  • Will share on-call duties as needed. REQUIRED POSITION QUALIFICATIONS

  • Master's Degree from an accredited School of Nursing program (MSN)

  • Current license to practice as a registered nurse in the State of Washington

  • Minimum of 7 years of nursing experience; 5 years of clinical experience working in a Care Coordination role

  • ACM or CCM Certification preferred and required within one year of employment

  • Leadership experience in a healthcare setting preferred.

  • Strong data management skills, preferred.
