Responsibilities

  • Case management of families to assist with overcoming the barriers and family stabilization using the Family Development strengths based approach.

  • Complete comprehensive assessments of family strengths and needs using agency tools and best practices. Develop and monitor goals and milestones with family participation.

  • Conduct site visits and model best practices to our community providers to ensure alignment with organizational standards.

  • Maintain regular contact with partnering providers to offer support, model best practices, and promote quality interactions with children and their families.

  • Ensure comprehensive access, including referral and follow-up to all SCAP services, community-based agencies, government agencies and others, to fully integrate, streamline and coordinate family services.

  • Conduct timely data collection and entry in data management system; maintain case notes and demographics so reports are retrievable and up-to-date. Timely and accurate completion of all required paperwork.

  • Promote parent learning to enhance parent-child relationship. Facilitate networks and group activities that support families’ strengths, interests, and needs.

  • Conduct advocacy on behalf of families in a solution-focused manner and represent SCAP in a positive, professional way.

Qualifications

Requirements

  • Bachelor’s Degree in related field with two (2) years directly related experience. Master’s Degree preferred. Family Development Credential preferred.

  • Knowledge of community based services, public assistance and other community resources and demonstrated skills in accessing these services appropriately. Must have the ability to learn quickly the resources available in Schenectady County, and be able to appropriately refer in-house and within the community.

  • Computer literacy required and the ability to consistently upgrade skills to remain up to date on the use of technology in the workplace.

  • Knowledge and understanding of issues related to individuals and families in poverty. Ability to build upon the strengths of those we serve with diverse racial, cultural, social, educational and economic backgrounds, including high risk populations, to provide high-level services and lasting solutions to fit each individual’s and family’s needs.

  • Demonstrated written and verbal communication skills and the ability to work as part of an agency wide team.

  • Flexible work schedule as required to meet the needs of the program and customers.

  • Must possess a valid New York State driver’s license, have regular access to a car and be willing to travel to off-site locations.

  • Applicants must be fully vaccinated against COVID-19 and show proof of vaccination [or receive a first vaccination by their first day]. Exemptions may be available on the basis of disability, medical contraindications to the vaccine, pregnancy or pregnancy related medical conditions and for religious reasons. Any requests for an exemption will be reviewed by the SCAP Human Resources Director and determined on a case by case basis.
