ACT Team Counselor - Part Time We are seeking a community Counselor for 20 hours per week. The hours can be flexible within the client service hours of Monday-Friday, daytime hours.Responsibilities of the ACT Team Counselor - Part Time: The ACT Team Counselor is a member of the ACT multidisciplinary team, and has the experience to assess, plan, develop, coordinate, and provide treatment and support services to program clients with a diagnosis of severe mental illnesses. These services will be provided in the client’s home, community, or office. Specific duties:- Using a collaborative team approach, conduct comprehensive assessment of psychiatric and substance abuse histories, and mental status under the supervision of the Director of ACT or ACT Assistant - Program Director.- Use strength-based methodologies, continually instill hope, develop, and maintain trusting and supportive relationships with clients, families, other providers, and the community. - In collaboration with ACT Team staff members, develop and implement person-centered individual treatment plans aligned with OMH guidance.- Provide supportive service, treatment, case management services, community agency advocacy and linkage services to all clients served by the team.- Perform supportive counseling and case management to individuals, family, and significant supports related to the client and his/her care needs. These services will take place in the client’s home, community and/or the office.- Cofacilitate therapeutic groups as for individuals and/or families based on needs and wellbeing of the individuals.- Regularly attend Team Meeting for treatment review and planning, program updates, and planning, intake status, and participate in on-going client care planning, program operations and team needs and concerns. - Provide transportation to clients when necessary, using agency vehicles. Mohawk's Assertive Community Treatment - ACT Team:This unique team is an evidence-based treatment program that utilizes a mobile, multidisciplinary team of staff to deliver flexible, comprehensive treatment and rehabilitation services to individuals residing in the community with a severe mental illness.The program’s goal is to help individuals achieve stability within their community.Services are intensive and non-traditional in nature and provided by a team in the community including in the client’s home.Outreach is a key component of the service model.Strong team support and availability for collaboration.RequirementsSkills/Qualifications for the ACT Team Counselor - Part Time:1. Bachelor’s degree in human services OR an associate degree in human services with a CASAC or other relevant credentials.2. Three years’ experience in human service. Specific experience in mental health or substance use disorder preferred.3. Skills and competence to establish and maintain supportive and trusting relationships with clients, families, and all community providers. Also, competence in direct care provision and demonstrate competency in community safety and community psychiatry methodologies.4. Ability to respect clients’ rights and personal preferences in treatment,5. Mohawk’s programs and offices are multi-level with stairs only. All employees must be able to travel to all levels of the facility in a safe and reasonable time frame.6. A valid and insurable driver’s license.BenefitsSalary and Schedule for the ACT Team Counselor - Part Time:- $30.00/hour; 20 hours per week; $31,200/annual equivalent; prorated fringe benefits.- Schedule: 20 hours per week. Work hours are within the client service hours of Monday-Thursday, daytime hours.
